This COFF will provide useful feedback to DOST-FNRI and will help in identifying areas for improvement relating to the products and services we provide.

In compliance with Republic Act No. 10173 also known as the Philippine Data Privacy Act of 2012, DOST-FNRI recognizes its responsibility to respect and protect the data of its clients that it collects, uses, organizes, records, updates, consolidates, or deletes.

The DOST-FNRI has instituted appropriate measures to ensure the protection of its participants' data:

(1) Personal data obtained from this form is entered and stored within the organization’s authorized database and will only be accessed by authorized DOST-FNRI personnel.

(2) Such information collected and stored from this registration form shall only be used for the following purposes: processing and reporting of documents related to the event, issuance of the certificate of participation/ completion, directory announcements/promotions of events, programs, courses and other activities offered/ organized by the DOST-FNRI.
